Clinical Health Coach® (CHC) is America's leading clinically focused, performance oriented health coach training program, developed in partnership with primary care providers. It was created in 2008 by the Iowa Chronic Care Consortium. NCQA is a private, nonprofit organization dedicated to improving health care quality. Celebrating its 25th anniversary in 2015, NCQA accredits and certifies a wide range of health care organizations.
