The Commission for Case Manager Certification
The Commission for Case Manager Certification® is the first and largest nationally accredited organization that certifies case managers. AAAHC (Accreditation Association for Ambulatory Health Care)
Believing that health care quality and patient safety are best evaluated at the point of care, AAAHC launched the first on-site survey for accreditation of Medical Homes in 2009. We have since added Medical Home On-Site Certification as a focused form of recognition for Medical Homes and an alternative to accreditation.

AAAHC currently accredits more than 6,000 organizations in settings that include group practices, community-based, tribal, and student health centers, military health facilities, and ambulatory and office-based surgery centers. Clinical Health Coach® (CHC)
Clinical Health Coach® (CHC) is America's leading clinically focused, performance oriented health coach training program, developed in partnership with primary care providers. It was created in 2008 by the Iowa Chronic Care Consortium. Since its inception, nearly 1,000 medical professionals (physicians, nurses, medical assistants, social workers, registered dietitians, pharmacists, health educators) from primary care, hospital and public health organizations have completed our onsite or online training programs. NCQA is a private, nonprofit organization dedicated to improving health care quality. Celebrating its 25th anniversary in 2015, NCQA accredits and certifies a wide range of health care organizations. It also recognizes clinicians and practices in key areas of performance.
