GRANTORS & EXHIBITORS

We thank the following Grantors and Exhibitors for their support in helping to make this conference possible.

GRANTORS
BRONZE

Davis Wright Tremaine
Those of us in the Davis Wright Tremaine health care practice group devote nearly all of our time to industry-specific matters. This allows us to develop niche practices within the field that serve needs unique to the health care market. We also understand the business issues and regulatory developments that affect health care delivery and compliance, and as a unified team we can match the attorney with the niche expertise that best addresses client needs. For more information visit our website at www.DWT.com.

EXHIBITORS

Clinical Health Coach® (CHC)
Clinical Health Coach® (CHC) is America's leading clinically focused, performance oriented health coach training program, developed in partnership with primary care providers. Since its inception, more than 2,000 medical professionals (physicians, nurses, medical assistants, social workers, registered dietitians, pharmacists, health educators) from primary care, hospitals, ACO's and public health organizations have completed our onsite or online training programs. Successful graduates deliver improved patient-centered care through evidence-based health coaching and communications skills. They support the Patient-Centered Medical Home principles by applying population health strategies to identify and provide proactive health coaching and care management for patients at highest risk for hospitalization or emergency room services. They influence positive patient health outcomes and drive improved quality measures in patient experience.

For more information, visit our website at www.clinicalhealthcoach.com or contact Kathy Kunath @ 515-971-3234.

The Compliance Team
Exemplary Provider® PCMH Accreditation

Introducing the 1st Patient-Centered Medical Home (PCMH) accreditation model that builds upon existing day-to-day primary care practice routines while incorporating simplified measures to account for today's payment transformation realities. The Compliance Team's operations-based Exemplary Provider® accreditation works with existing provider business models and meets the PCMH Joint Principles put forward by the American Academy of Family Physicians, American Academy of Pediatrics, American College of Physicians and the American Osteopathic Association.

Process simplification is the key. Providers gain clarity in an accreditation program modeled after The Compliance Team's Medicare-deemed Exemplary Provider® accreditation program for Rural Health Clinics. Expert-led implementation webinars guided by our plain language Safety-Honesty-Caring® quality standards help providers streamline their patient care practices and improve overall operational efficiencies without disrupting their existing everyday work environments to do so. TCT's program is also scalable and can be customized to include health maintenance, diagnostic preventative screening and multi-specialty medical services.
