John Iglehart
Founding Editor, Health Affairs; National Correspondent, New England Journal of Medicine, Washington, DC (Co Chair)

    Speaker Bio

    John Iglehart is editor-in-chief of Health Affairs, a bimonthly policy journal he founded in 1981, led until September 2007, and resumed in 2013 with the departure of Susan Dentzer. The journal is published by Project HOPE, a not-for-profit international health education organization. He is also a national correspondent for The New England Journal of Medicine. Before 1981, Iglehart served for two years as a vice president of the Kaiser Foundation Health Plan and director of its Washington, D.C. office. During the decade 1969 to 1979, Iglehart held a variety of editorial positions, including the editorship of National Journal, a privately published weekly on federal policy. He was elected to membership in the Institute of Medicine (IOM) in 1977 and served on its Governing Council for six years (1985-1991). He also is an elected member of the National Academy of Social Insurance and serves on the Advisory Board of the National Institute for Health Care Management. He holds a degree in journalism from the University of Wisconsin-Milwaukee and has been a journalist-in-residence at Harvard University.

Lessons from 20 Years of Primary Care Support

Ronda Kotelchuck
Chief Executive Officer, Primary Care Development Corporation (PCDC), New York, NY

    Speaker Bio

    Ronda Kotelchuck is the founding CEO of the Primary Care Development Corporation (PCDC). She served on the original workgroup that conceptualized, designed and established PCDC in 1993. Under Ms. Kotelchuck's leadership, PCDC has financed more than 100 primary care projects totaling $485 million and modernized more than 925,000 square feet of space. PCDC has also provided Performance Improvement training and coaching to nearly 900 primary care organizations throughout the US.

    Prior to PCDC, Ms. Kotelchuck previously served as Vice President for Corporate Planning and Intergovernmental Relations for the NYC Health and Hospitals Corporation. Ms. Kotelchuck received an MA in Regional Planning from Cornell University.

Medical Homes and the States

Marie Maes-Voreis, RN, PHN, MA
Director Health Care Homes, Department of Health, State of Minnesota, St. Paul, MN

    Speaker Bio

    Marie Maes-Voreis is Director of Health Care Homes for the State of Minnesota. She has led the implementation of Health Care Homes legislation for the Minnesota Department of Health and the Minnesota Department of Human Services as a joint government private sector project since 2008. She is also the State's Co-Lead for the community integration and practice transformation work of the Minnesota Accountable Health Model-SIM Minnesota testing grant. Previously she worked as a Director of Primary Communication systems at Park Nicollet Health Services in St. Louis Park, MN and Director of the Family Medicine clinics at Hennepin County Medical Center (HCMC) in Minneapolis. She managed successful implementation of a medical home using a multi-disciplinary team care model in family medicine and has extensive experience with implementation of the EPIC electronic health record. Marie has a Master's in Health and Human Services Administration from St. Mary's University and a BSN from the College of St. Benedict.
Lisa M. Letourneau, MD, MPH
Executive Director, Maine Quality Counts, Manchester, ME

    Speaker Bio

    Dr. Lisa Letourneau serves as the Executive Director of Maine Quality Counts, a regional health improvement collaborative, and serves as physician champion for several quality improvement efforts, including the Maine Aligning Forces for Quality initiative and the Maine Patient Centered Medical Home (PCMH) Pilot. Dr. Letourneau is a graduate of Brown University and the Dartmouth-Brown Program in Medicine and is a board-certified internist who practiced emergency medicine for seven years before beginning her work in clinical quality improvement.

    Dr. Letourneau holds a Master's degree from the Harvard School of Public Health, and has a particular interest in helping to build connections between public health and clinical care, and the role of physicians in helping to develop and lead health improvement efforts.
Mary Takach, BSN, MPH
Senior Program Director, National Academy for State Health Policy (NASHP), Portland, ME (Moderator)

    Speaker Bio

    Mary Takach is a senior program director at the National Academy for State Health Policy where she focuses on issues related to patient-centered medical homes, federally qualified health centers, and delivery system reform. Her current projects include directing two Commonwealth Fund grants -- a multi-year project helping states advance medical homes and a project to build a Medicaid "Accountable Care" central resource center; a Cooperative Agreement from HRSA to build partnerships with Medicaid Directors and safety net providers; a CMS contract to analyze state policy implications for the evaluation of the Multi-Payer Advanced Primary Care Practice Demonstration; and a PCORI grant to develop a "Patient-Centered Outcomes Research and Comparative Effectiveness Research Roadmap for State Policymakers." She has worked on Capitol Hill as a Legislative Assistant for two Congressmen and in a wide variety of clinical settings as a Registered Nurse. She holds a MPH degree from The Johns Hopkins Bloomberg School of Public Health and a BSN degree with honors from Northeastern University.

Regional Extension Center Resources for Medical and Health Home IT

Thomas Novak
Director of Delivery System Reform, Office of the National Coordinator for Health IT, U.S. Department of Health and Human Services, Philadelphia, PA

    Speaker Bio

    As Director of Delivery System Reform in the Office of Provider Adoption and Support (OPAS), Thomas Novak coordinates the aggressive support - via 62 Regional Extension Centers nationwide - for the implementation of Meaningful Use Stage 2 with an emphasis on coordination with other HHS priorities such as Health Information Exchange, EHR Privacy & Security, Consumer Engagement, and a particular emphasis on alignment with new care models such as Patient-Centered Medical Homes & Accountable Care Organizations. He also leads the support efforts for ICD-10 and serves on several committees supporting Correctional Health initiatives.

    Prior to coming to ONC, he was CMS's Medicaid HITECH lead for the eastern United States and the national lead on eligibility policy for the Medicaid EHR Incentive Program. He wrote the portions of the Meaningful Use Stage 2 regulation that pertain to eligibility and children's hospitals. Thomas has provider experience having been the Director of Quality for the largest HIV agency in Wisconsin where he led the agency's successful efforts to attain a Level 3 Recognition as a Patient Centered Medical Home.

Why Employers and Unions Love the Medical Home

Robert J. Krzys
Attorney, New Hartford, CT

    Robert J. Krzys has focused his law practice on representing labor unions. Since 1990, he has represented the coalition of Connecticut's State Employee Unions in bargaining with the State of Connecticut over health insurance coverage for Connecticut's active and retired employees, approximately 225,000 people. In 2011, in the midst of negotiations to restructure health and pension benefits in order to help address a state budget deficit, the Union coalition endorsed a Value Based Insurance Design (VBID) approach to existing health benefits. Attorney Krzys played a leading role in constructing and presenting and negotiating the final agreement. That agreement implemented a comprehensive VBID design which modified and complemented the existing offerings to active employees and to those employees who retired after October 1, 2011.

    Ninety-eight (98%) of employees enrolled and have therefore participated in the new Health Enhancement Program (HEP). The HEP program is designed to greatly increase screenings, provide counselling to those with chronic diseases and to incent people to use the most valuable services offered by the health plan by reducing and eliminating certain co-pays and by paying a cash bonus to those chronics who comply with an education and counselling program. Together with co-panelist Kevin Lembo, Comptroller of the State of Connecticut who is responsible for the administration of the HEP program, they will share the Connecticut experience and speak about its relationship to patient centered medical homes.
The Hon. Kevin Lembo

    Speaker Bio

    Kevin Lembo was elected Connecticut's State Comptroller in 2010 following 20 years of public service and advocacy. A native of Paterson, NJ, Lembo first came to public service as an independent advocate. He helped design and implement an innovative long-term home care program in New York that successfully prevented premature and permanent admissions to nursing facilities, and he served as program director for an AIDS education, prevention and primary care program. Lembo was appointed Connecticut's first state Healthcare Advocate in 2004 where he spent years helping thousands of residents navigate the complexities of the health care system; advocated for patients denied coverage or treatment; and returned millions of dollars to consumers.

    As State Comptroller, Lembo continues to lead the charge for more affordable and quality health care, and has been hailed by advocacy groups and the media as a "champion of transparency" for his efforts to promote public access to vital state financial information.

John Bray
Chief Executive Officer, Vanguard Furniture, Hickory, NC

    Speaker Bio

    John Bray is Chairman and CEO of Vanguard Furniture Company, a 45-year old furniture manufacturer in Hickory, NC. Vanguard is a custom order upholstery manufacturer with over 500 employees and sells its products to retailers and designers in both domestic and international markets. For the last 25 years, John has served on the Board of Directors of Frye Regional Medical Center. FRMC is a 350 Bed Hospital in Hickory. He is also a member of the Board of Trustees for the local community college -- Catawba Valley Community College in Hickory. John was a Captain in the Marine Corps and was awarded a Bronze Star while serving in Viet Nam. He is a graduate of Wake Forest University with a Bachelor of Business Degree.

Bruce Sherman, MD, FCCP, FACOEM
Medical Director, Employers Health Coalition, Canton, OH (Moderator)

    Speaker Bio

    Dr. Bruce Sherman as Medical Director with the Ohio-based Employers Health Coalition, where he brings health management and value-based purchasing strategies to member employer members. He also serves as the consulting Corporate Medical Director for Wal-Mart Stores, Inc. In this role, he supports the development of integrated, value-based health and productivity management strategies for the organization's associates and family members in the US. Dr. Sherman has particular interests in the areas of the business value of health and innovative employer approaches to healthcare delivery. Sherman is a member of the Patient-Centered Primary Care Collaborative, where he is involved with the Center for Employer and Purchaser Engagement. He is also a member of the leadership Board of the Integrated Benefits Institute. A frequent speaker at national venues, he has presented workforce health management strategies to diverse audiences, and has published numerous related articles.

    Speaker Bio

    Dr. Joseph Scherger is Vice President for Primary Care and Academic Affairs at Eisenhower Medical Center in Rancho Mirage, California. Dr. Scherger is Clinical Professor of Family Medicine at the University of California, San Diego School of Medicine (UCSD), and at the Keck School of Medicine at the University of Southern California (USC). Dr. Scherger's main focus is on the redesign of office practice using the tools of information technology and quality improvement.

What We've Learned, Where We Go from Here

Marci Nielsen, PhD, MPH
Chief Executive Officer, Patient-Centered Primary Care Collaborative, Washington, DC

    Speaker Bio

    Dr. Marci Nielsen is Chief Executive Officer of the Patient-Centered Primary Care Collaborative (PCPCC). She previously served as Vice Chancellor for Public Affairs and Associate Professor within the Department of Health Policy and Management at the University of Kansas School of Medicine, where she was twice awarded Professor of the Year by her students. She also served on the Board of Directors of the Health Care Foundation of Greater Kansas City, TransforMED, and the MidAmerica Coalition on Health Care. She also has been a committee member on the Institute of Medicine's Leading Health Indicators for Healthy People 2020 and Living Well with Chronic Illness: A Call for Public Health Action. She serves as the first public (non-physician) member on the American Board of Family Medicine. Prior to rejoining the KU faculty, she was appointed by then-Governor Kathleen Sebelius to run the Kansas Health Policy Authority (KHPA). At the national level, she has worked as a legislative assistant to U.S Senator Bob Kerrey (D-Nebraska) and as the health lobbyist and assistant director of legislation for the AFL-CIO. She earned her MPH degree from George Washington University and her PhD in Health Policy and Management from the Johns Hopkins School of Public Health.
